When you refer a patient to Windsor-Laurelwood Center for Behavioral Medicine, it helps our Access Center immensely when you provide basic information regarding the prospective patient. Please review the information so you may assist our staff in making the admissions process go smoothly for the individual that you are referring.

Inpatient Criteria:

Individuals of all ages beginning at age 3
Psychiatric or addiction diagnosis
Voluntary and involuntary admissions are accepted

Please provide details regarding the reason for request for admission. In addition,
Windsor-Laurelwood Center for Behavioral Medicine requires the information below.

Demographic:

• Patient Information such as name, address, primary contact and social security number
• Insurance Information including Private Insurance, Medicare or Medicaid (ages under 20 and over age 65)

• Medical Assessment
• Psychiatric History
• Current Psychiatric Medications

• Power of Attorney (if applicable)
• Guardianship Paperwork (if applicable)
